Privacy policy
Your information, handled with care.
How United Health Professionals handles the personal information you give us through this website.
Who we are
United Health Professionals Pty Ltd (ABN 69 649 956 776), Strathtulloh VIC 3338. In this policy, "we" and "us" means United Health Professionals.
This policy covers information collected through this website. Information about the supports we provide is handled under your service agreement and our obligations as a registered NDIS provider.
What we collect
When you send an enquiry we collect your name, phone number, email (if given), suburb, who the support is for, the supports you're interested in, the best time to call and any message you write.
When a support coordinator sends a capacity check we collect their name, organisation, phone, email, the participant's suburb, days and times, funding type, worker preference, supports needed and any notes.
Some of these details, such as the supports needed, are health information, so we only ask for what we need to respond and we collect them with your consent. Please don't include diagnoses, medical details, participants' names or NDIS numbers in these forms. We'll ask for what we need when we speak with you.
Why we collect it
To reply to your enquiry, check whether we can provide the supports you need, and arrange a time to meet. We don't sell your information and we don't add you to marketing lists.
Who sees it
- Our team at United Health Professionals.
- Contractors who help us respond to enquiries and arrange first visits on our behalf. They use your details only for that.
- Service providers that host the website and deliver enquiries to us. These providers may store information outside Australia, including in the United States.
Cookies
This website doesn't use advertising cookies. Our hosting provider keeps standard server logs for a limited time to keep the site running and secure. We use Vercel Web Analytics to count visits and see which steps of our enquiry form people reach. It doesn't use cookies, can't identify you and never receives what you type into our forms.
Accessing or correcting your information
You can ask what information we hold about you, ask us to correct it, or ask us to delete it. Email info.unitedhealthprofessionals@gmail.com or call 0416 851 984.
Complaints
If you have a concern about how we've handled your information, please contact us first so we can fix it. If you're not satisfied with our response, you can contact the Office of the Australian Information Commissioner at oaic.gov.au or 1300 363 992. For complaints about NDIS supports, see feedback and complaints.
